Job Description:

SUMMARY:

The Clinical Care Coordinator is a Registered Nurse who provides care coordination, medical and disease education and outreach services to patients. When identified as Coordinated Care Team Lead, this position will convene and facilitate Interdisciplinary Care Team meetings and collaborate with team Licensed Social Workers and Community Health Workers. This position requires licensure in a health discipline that allows the professional to conduct an assessment independently within the scope of practice of their discipline.

 

KEY RESULT AREAS (KRAs)

  • Completes Health Risk, Disease Specific and Clinical Risk Assessments.

  • Identifies care needs and goals based upon:

  • Assessment findings

  • Analysis of claims data

  • Provider input

  • Patient and parent/caregiver expressed goals

  • Establishes Care Plan, in conjunction with patient and parents/caregivers and key providers. Plan shall identify:

  • Essential primary, specialty, and ancillary services and providers

  • Needed social support and services and identified community-based organizations that can address those needs

  • Educational needs of patient and/or caregivers to enhance self-management and appropriate utilization of providers to address psychological, developmental and/or medical conditions

  • Potential barriers to meeting goals and plan for mitigating barriers

  • Individualized disaster preparedness plan

  • Convenes and facilitates Interdisciplinary Care Team (IDCT) meetings to ensure communication and coordination between patient, parent/caregivers, and key healthcare and support service providers.

  • Monitors clinical and functional status of patients through regular face to face contact and convenes IDCT meetings on an ad hoc basis to address gaps in care or barriers to successful implementation of Care Plan.

  • Maintains and ensures the confidentiality of all Personal Health Information (PHI) collected and disseminated, in accordance with HIPAA requirements.

  • Provides patients and parents/caregivers with education required to care for child in home/community.

  • Directs, as appropriate, the Community Health Worker/Patient Engagement Rep with regard to:

      • Scheduling appointments

      • Linking patient and family/guardian to community based organizations and services

      • Follow up with/support for patient and family/guardian

      • Communication with/support for providers

  • Participates in relevant continuing education offerings to enhance knowledge base needed to oversee care for specialty populations.

  • Participates in all program specific training programs.

  • Adheres to the policies and procedures of University Clinical Health

  • Maintains strict confidentiality of client, company and personnel information

  • Demonstrates a strong commitment to the mission and values of the organization

  • Adheres to company attendance standards

  • Performs other duties as assigned

Experience and Skills:

  • An Associate’s Degree in Nursing is required; a Bachelor’s degree in a health related field is preferred.

  • A minimum of two (2) years critical care experience or five (5) years of general nursing experience.

  • A minimum of two (2) years of Case Management and/or Disease Management experience is strongly preferred.

  • Certified Case Management (CCM) Certification is required within four (4) years of employment. Other certifications recognized by URAC are also acceptable.

  • Current RN State licensure.

  • Excellent interpersonal, organizational, and analytic skills required to accomplish assessment calls and coordination/prioritization of interventions

  • Knowledge certain skill, procedures and processes.
